Transition
Your child will no
longer be eligible for JCDC services at three years of
age. A transition meeting
will be scheduled approximately three (3) months before your
child’s third birthday, so that you may meet with
a representative from the Jordan School District or Canyons
School District to talk about
your child’s needs and concerns. In accordance with
the State Board of Education Policy, and the Utah Department
of Health, a team of professionals will determine if your
child is eligible for services at age three. Even if your
child is eligible and services are recommended, it is not
mandatory for your child to participate. If your child is
not eligible to receive school district services, JCDC staff
will provide you with alternative suggestions and resources.
Release From Program
When your child is no longer demonstrating
delays in any developmental areas, s/he will not be eligible
for continuing early intervention services. However, with
your permission, your child can be placed on the JCDC Tracking
and Monitoring System, which will be described to you by your
Service Coordinator.
You are also free to discontinue JCDC
Services at any time if you so choose. If your family moves
outside of our service area, the JCDC staff will identify
a receiving program and provide information regarding current
IFSP services.
