The Individualized Family Service Plan
(IFSP) is the most important document you will sign while
in our program. The IFSP is developed at a meeting, involving
you and the professionals that work with your child.
The
IFSP identifies services and outcomes for your child
and family
for the next 12 months. During this meeting, we will discuss
your concerns and needs regarding your child or family
situation.
The number and type of visits/services are determined by
the child’s needs, recommendations of therapists, service
coordinators, and family input. The IFSP is reviewed and updated
to meet your child’s needs throughout the year. We
will develop a new IFSP with you annually in an effort
to help
your child reach his/her developmental potential.
