ATTENDANCE
As a partner with the JCDC
Staff, you are the person who has the greatest impact on
your child’s
development and progress. Please help us to serve you by
keeping your appointments as scheduled. Please notify the
program at
least 24 hours prior to the appointment if you are unable
to meet at the pre-arranged date and time. If you do not
notify the program within 24 hours you will be billed for
that visit
when appropriate.This does not
include
emergency cancellations due to sickness, accidents, or
other conditions not within your control. Please
call 801-567-8510 to change or cancel appointments.
Action To Be Taken Following Three “No Show” Appointments:
A “no show” is when a family has an appointment
they can’t keep and they don’t contact us to cancel.
After three (3) consecutive months of “no shows” for
scheduled visits (other than absences or emergencies), the
following will apply:
· An IFSP meeting will be scheduled
to determine if the family wants ongoing services.
· If the family does not attend the scheduled IFSP
meeting, a letter will be sent to the family requesting that
they contact the JCDC if they want further services.
· If the family does not contact the JCDC for continuation
of services, the child will no longer receive IFSP services,
but will be placed on a tracking and monitoring system.
ILLNESS
Many of our children are very vulnerable to illness; therefore,
the JCDC follows a very strict health policy. If your child
has any of the following, please call to cancel your session
for that day:
*fever
* vomiting *severe cough *unusual rashes *eye infection
*draining ear *diarrhea * itchy body or scalp *excessive
nasal discharge
